Modality Biophysical Mechanism Clinical And Diagnostic Utility
Diffusion-Weighted Imaging (DWI) & ADC Detects restrictions in random Brownian water motion caused by failure of the ATP-dependent Na+/K+ pump and intracellular cytotoxic edema. Gold standard for hyperacute ischemia (within hours); shows bright hyperintensity on DWI with reciprocal dark hypointensity on ADC maps. Differentiates acute from chronic lesions.
Magnetic Resonance Angiography (MRA) & Venography (MRV) 3D Time-of-Flight imaging maps vascular flow without utilizing contrast injections. MRA evaluates arterial patency and mechanical cut-offs (e.g., Middle Cerebral Artery). MRV is mandatory for suspected Cerebral Sinovenous Thrombosis (CSVT), identifying filling defects.
Susceptibility-Weighted Imaging (SWI) Uses high-resolution gradient-echo sequences to detect local magnetic distortions from paramagnetic blood degradation products. Highly sensitive for diagnosing petechial hemorrhages, microbleeds, and secondary hemorrhagic transformation via a distinct "blooming artifact".
Arterial Spin Labeling (ASL) Perfusion Uses magnetically inverted inflowing arterial blood water protons as an endogenous tracer, avoiding gadolinium contrast. Measures regional Cerebral Blood Flow (CBF); differentiates ischemic penumbra (hypoperfusion) from tissue reperfusion (luxury perfusion).
Diffusion Tensor Imaging (DTI) Maps microscopic orientation and directionality of water diffusion (anisotropy) along organized white matter tracts. Evaluates structural integrity of descending motor pathways; decreased Fractional Anisotropy (FA) in the Posterior Limb of the Internal Capsule (PLIC) strongly predicts hemiplegic cerebral palsy.
Magnetic Resonance Spectroscopy (MRS) Quantifies relative concentrations of specific biochemical metabolites within targeted brain tissue. Severe acute ischemic injury shows a substantial drop in N-acetylaspartate (neuronal loss) and a prominent inverted doublet peak of Lactate (anaerobic glycolysis).
