Definition

The Open Vial Policy (OVP), also known as the Multi-Dose Vial Policy (MDVP), is a programmatic guideline endorsed by the World Health Organization (WHO) and implemented by the Ministry of Health and Family Welfare (MoHFW) under the Universal Immunization Programme (UIP). It permits the safe utilization of opened multi-dose vials of specific liquid vaccines in subsequent immunization sessions for up to 28 days, provided strict criteria concerning cold chain maintenance, sterility, and vaccine vial monitors are fulfilled.

Rationale and Objectives

Criteria for Applicability

An opened multi-dose vial can be safely utilized in subsequent sessions for up to 28 days ONLY if it fulfills all four of the following absolute prerequisites:

  1. Expiry Date: The explicit expiry date printed on the vaccine vial has not passed.
  2. Cold Chain Maintenance: The vaccine has been continuously stored under appropriate and documented cold chain conditions (between 2 to 8 degrees Celsius).
  3. Vaccine Vial Monitor (VVM) Integrity: The VVM is intact, attached directly to the vial, and has not reached the discard point. It must be in Stage 1 or Stage 2 (where the inner square is lighter than the outer reference circle).
  4. Sterility and Physical Integrity: There is no visible evidence of biological contamination, no floating particles, the septum remains uncompromised, and the vial has not been submerged in water.

Vaccines Included in OVP

These are typically liquid formulations containing specific preservatives (such as thiomersal) that actively inhibit bacterial and fungal proliferation. They can be used up to 28 days after the first puncture:

Vaccines Excluded from OVP

These vaccines either lack preservatives, are live attenuated lyophilized formulations that rapidly lose potency, or are highly susceptible to contamination post-reconstitution. They must be discarded within 4 hours of opening/reconstitution, or at the conclusion of the immunization session, whichever is earlier:
